RN Clinical Liaison

We’re looking for a *Registered Nurse* to join our team.

As part of our exceptional team, you will report directly to the Executive Director.
Responsibilities include: * Serve as point person to ensure referrals and intake processes are completed efficiently and timely in coordination with area facilities.
* Provide education regarding our hospice services to area providers.
* Complete patient assessments and admissions processes as appropriate.
* Maintain thorough clinical record documentation.
* Participate in company quality assessment and performance improvement programs.
Job Type: Full-time Pay: From $31.
